Work Environment

This position is primarily onsite, with therapists conducting visits in patients' homes, thus offering flexible scheduling options to meet patient needs. Our operating hours are from 7:30 am to 3:30 pm or 8:00 am to 4:00 pm, Monday through Friday.

Key Responsibilities

As a Home Care Physical Therapist, your core responsibilities will include:

• Assessing and treating patients in their home environments, tailoring therapy to individual needs and circumstances.
• Establishing comprehensive treatment plans that incorporate a variety of therapeutic techniques, such as exercise, stretching, functional training, and hands-on modalities to alleviate pain and enhance mobility.
• Documenting each patient's treatment progress meticulously, ensuring responses to therapy are recorded accurately to inform ongoing care decisions.
• Continuously reevaluating therapeutic goals based on patient feedback and clinical outcomes, and adjusting treatment plans accordingly.
• Providing education and training for patients, caregivers, and family members on the use of assistive devices, therapeutic exercises, and home safety measures.
• Maintaining up-to-date knowledge in physical therapy practices through participation in workshops and professional organizations.

Patient Population / Client Focus

Your work will primarily involve helping children and adults who are recovering from various conditions, including post-surgical rehabilitation, sports injuries, and chronic health issues. You will focus on not just physical recovery but also on empowering patients through education and self-management strategies, all while creating a comfortable and supportive atmosphere for healing.

Qualifications

To be considered for this role, candidates must possess the following qualifications:

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid and active state license in physical therapy.
• At least two years of experience in a clinical setting, demonstrating proficiency in therapeutic practice.
• Strong written and verbal communication skills, essential for providing clear instructions and documenting patient care effectively.
• Proficient computer skills for managing electronic patient records and communication.
• Ability to adapt and demonstrate agility in different clinical scenarios.

Preferred Experience

While two years of experience is required, prior experience in home care settings is preferred but not mandatory. Candidates who are newly graduated and eager to learn through mentorship are also encouraged to apply.

Compensation and Benefits
