Key Responsibilities\n

• Conduct thorough assessments and develop individualized treatment plans based on collected data and client feedback.

• Maintain consistent communication with supervisors, direct care staff, families, and guardians to provide updates on client progress, ensuring all stakeholders are informed.

• Schedule and coordinate virtual appointments, home visits, and observation sessions to monitor patient advancements and efficacy of the treatment plan.

• Ensure that all clinical interventions and program plans are implemented effectively and in line with best practices.

• Participate proactively in interdisciplinary team meetings to discuss client progress and collaborate on care strategies.

• Document and complete monthly and quarterly progress notes to track client outcomes and adjust treatment as necessary.

• Organize and conduct training sessions to equip staff with the necessary skills to implement program plans and maintain accurate documentation.

• Conduct follow-up interventions, including additional home visits and observations, continuing to provide support tailored to individual client needs.

• Act as a liaison between clients and the broader mental health community to facilitate access to resources and support systems.

• Contribute to the development of training materials, procedures, and best practices to enhance program delivery.

Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from $90,000 to $100,000 annually, depending on experience and caseload capacity.
