Key Responsibilities\n

• Design and coordinate individualized support plans to help clients navigate through their challenges and recover from dependencies.

• Conduct comprehensive interviews and assessments to understand clients' current situations, needs, strengths, and potential barriers to success.

• Set measurable and achievable goals for each client's progress, treating every case as a unique unit of care.

• Monitor clients’ progress through regular follow-up sessions, adjusting treatment plans and strategies as necessary based on ongoing evaluations.

• Provide crisis intervention during sessions, helping clients to stabilize their emotions and develop coping strategies.

• Document case details meticulously, maintaining accurate records and regularly reporting on client statuses to ensure continuity of care.

• Participate in case conferences, sharing insights and collaborating with team members and, when necessary, providing expert witness testimony in court.

• Act as a key contact for clients, liaising with various agencies to facilitate appropriate external referrals for additional services.

Qualifications\n

Compensation and Benefits\n
