Work Environment\n

This position is fully remote, providing flexibility that allows social workers to adapt their work environment while engaging with clients effectively. You will be part of a dedicated team of professionals who collaborate to ensure comprehensive care for our clientele. Although much of your work will be conducted from home, you will engage actively with clients in a hospice center setting, which emphasizes support and recovery during their most vulnerable times.

Key Responsibilities\n

• Conducting thorough assessments for clients experiencing significant mental health issues, identifying needs for tailored therapeutic interventions.

• Creating, implementing, and monitoring integrated care plans that link clients to essential services, ensuring the coordination of healthcare providers and family members.

• Providing direct therapy including individual, family, and group therapy to help clients develop coping strategies and achieve desired therapeutic outcomes.

• Conducting brief, evidence-based screenings and conducting follow-up assessments as needed.

• Identifying and reporting unsafe situations to maintain a secure environment for clients.

• Participating actively in team meetings to enhance service delivery, share insights, and foster a collaborative team spirit.

• Carrying an active caseload, ensuring timely progression of treatment and monitoring ongoing client engagement and adherence to care plans.

• Providing crisis intervention and support when required to address immediate client needs.

• Maintaining clear and timely documentation of client encounters, treatment plans, and progress reports within the required systems.

Patient Population / Client Focus\n

The primary focus of this position will be on clients residing in hospice care who are facing terminal illnesses and severe emotional distress. This role requires sensitivity and expertise in dealing with grief, loss, and complex family dynamics as clients navigate end-of-life care. Compensation and Benefits\n

The salary for this role ranges from USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
