Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, evaluating emotional, social, and mental health factors that impact their well-being.

• Develop and implement individualized care plans in partnership with clients and their families, addressing personal goals and long-term aspirations.

• Provide therapeutic counseling and emotional support to seniors, assisting them in coping with life changes, such as health challenges, loss, and transitional phases.

• Actively advocate for clients’ rights, ensuring they have access to necessary community resources, services, and support systems.

• Collaborate with healthcare professionals across various disciplines to coordinate comprehensive care, focusing on improving client outcomes.

• Conduct regular virtual check-ins to monitor the well-being of clients, assess living conditions, and identify any new challenges they may face.

• Facilitate open communication among family members regarding the care processes and address any conflicts that may arise in the management of care.

• Educate seniors and their families about available community resources, programs, and support groups aimed at enhancing social engagement and overall quality of life.

• Maintain meticulous records of client interactions, assessments, and progress documentation in adherence to best practices and regulatory standards.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of relevant experience in social work, preferably serving the geriatric population or in a community health center setting.

• In-depth understanding of social work principles, community resources, and ethical issues impacting the aging population.

Compensation and Benefits\n
