Key Responsibilities\n

• Independently manage a specific caseload of patients, conducting thorough evaluations to assess their social, emotional, financial, and cultural backgrounds, which are integral to their treatment plans.

• Design and implement individualized therapeutic interventions that address psychosocial barriers, ensuring timely transitions through the continuum of care.

• Act as a leader within the healthcare team, coordinating efforts related to protective services, life care decisions, and advance directives.

• Provide crisis intervention services, offering support to patients and families during critical situations while maintaining collaboration with the healthcare team.

• Facilitate education sessions for patients and families on available resources and referrals, enhancing their ability to navigate complex healthcare requirements.

• Supervise Social Workers, MSWs, interns, and volunteers, providing weekly guidance to enhance the team's effectiveness and adherence to departmental standards.

• Engage with community agencies to ensure appropriate resources are in place for patients, aiding in the prevention of hospital readmissions.

• Maintain a proactive approach to respond promptly to psychosocial service requests, executing treatment plans without delays.

• Coordinate care for child patients, ensuring all discharge needs align with the services provided by relevant community agencies.

• Participate actively in team rounds and case conferences to discuss patient progress and multidisciplinary care strategies.

Qualifications\n

Compensation and Benefits\n

The successful candidate will receive a competitive salary ranging from USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
