Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, contributing to the holistic care team.

• Conduct thorough patient intakes, focusing on diagnostic assessments, functional evaluations, and formulating individualized treatment plans in conjunction with primary care providers.

• Deliver brief but impactful therapeutic interventions for patients requiring immediate mental health services.
\n
• Advise primary care providers on the appropriate referral paths, determining whether patients should continue within the primary care setting or be directed to specialized mental health resources.

• Implement follow-up procedures to monitor patient progress, assessing the efficacy of treatment plans and making necessary adjustments.
\n
• Develop relapse prevention strategies to assist patients in maintaining stability and reducing the risk of future episodes.
\n
• Recognize 'at risk' patients and collaborate on preventive strategies to improve their overall mental health.
\n
• Maintain detailed documentation of patient progress, assessments, and treatment plans in compliance with health regulations.
\n
• Provide ongoing education and self-management strategies to empower patients in their recovery journey.

• Uphold the highest standards of ethical conduct as outlined by the Code of Ethical Conduct for Clinical Professionals.

• Participate in team meetings and provide consulting insight into behavioral health services and program evaluation.

• Assure patients comply with medical treatments suggested by primary care providers, providing coping strategies as needed.

Patient Population / Client Focus\n

You will be working with a diverse patient population, including individuals from various socioeconomic backgrounds and age groups. Patients may present with a range of mental health challenges, including anxiety, depression, trauma, and behavioral issues, all requiring a tailored approach to treatment.

Qualifications\n

• A Master’s Degree in Clinical Counseling is required.

• A current Licensed Professional Counselor (LPC) license is a must.
\n
• Extensive knowledge in mental health assessment, treatment planning, family therapy, group therapy, and child psychotherapy.

• Strong understanding of DSM-IV diagnoses and various behavioral therapy techniques.
\n
• Excellent communication skills and the ability to build rapport with patients and collaborate with a multidisciplinary team.

• Demonstrated ability to maintain clinical records and generate reports that reflect patient progress.

Preferred Experience\n

Compensation and Benefits\n
