Key Responsibilities\n

• Create personalized relapse prevention plans to help patients maintain stability and manage their conditions effectively.

• Identify and support at-risk patients, devising proactive measures to prevent the escalation of their mental health issues.

• Record all relevant patient progress and diagnostic information accurately in treatment charts, ensuring compliance with best practices and regulations.

• Engage in continuous communication with the primary care team, ensuring comprehensive care delivery for all patients.

• Facilitate changes in behavior for patients with co-occurring physical health issues, encouraging healthier lifestyle choices and self-management skills.

• Provide educational materials and resources to empower patients in their treatment journey and recovery process.

• Consult on behavioral services with clinic management and provide insights for outcome evaluation and program development.

• Assist patients in adhering to medical treatments prescribed by primary care providers, including strategies to help cope with medication side effects.

• Uphold a strict adherence to the Code of Ethical Conduct for Clinical Professionals and engage in any other assigned duties relevant to the role.

Qualifications\n
