Key Responsibilities\n

• Consult with physicians and other healthcare providers to report and discuss patient progress, needs, concerns, and any potential problems encountered during treatment.

• Maintain accurate and timely clinical records, including treatment and progress notes, in compliance with department policies to ensure thorough tracking of patient outcomes.

• Collaborate closely with the healthcare team to develop comprehensive treatment plans, including discharge plans based on thorough psychosocial assessments tailored to individual client needs.

• Conduct individual and group counseling sessions, providing crisis intervention and ongoing support to clients while assessing and treating a variety of emotional and behavioral disorders.

• Assist clients with discharge planning, ensuring continuity of care through effective care coordination with local service providers and community resources.

• Engage with clients' support systems, including family members, to discuss treatment plans and facilitate their involvement in the recovery process.
