This position is based in Minden, Nevada, a charming community surrounded by the breathtaking Sierra Nevada mountains. The geographic location provides an ideal setting for our home care services, allowing our Physical Therapists to work closely with patients in a familiar and comfortable environment. You will collaborate directly with patients, caregivers, and other healthcare professionals to design and implement effective treatment plans.

Key Responsibilities

• Conduct comprehensive physical evaluations to assess patients' range of motion, strength, balance, gait patterns, and functional limitations.
• Develop individualized treatment plans based on thorough assessments, incorporating specific therapy goals, tailored exercise routines, and appropriate modalities (such as heat therapy or manual therapy).
• Implement treatment strategies through targeted exercises aimed at enhancing strength, flexibility, balance, coordination, and pain management.
• Train residents in walking techniques and the use of assistive devices like canes and walkers, while focusing on fall prevention strategies and safety measures.
• Guide patients in performing activities of daily living (ADLs) such as dressing, bathing, and transferring, promoting independence and confidence.
• Educate residents and caregivers on correct exercise techniques, home exercise programs, and effective fall prevention measures to ensure continuity of care outside of therapy sessions.
• Maintain comprehensive patient records, including progress notes, treatment plans, and functional assessments to monitor advancements and adjust strategies accordingly.

Patient Population / Client Focus

As a Physical Therapist at Suncrest Physical Therapy, your primary focus will be on elderly patients who may face various challenges due to age and accompanying medical conditions. You will work closely with individuals recovering from surgeries or managing chronic conditions, ensuring they receive the individualized care and support necessary for their recovery and well-being.

Qualifications

• A valid Physical Therapy license in the state of Nevada is required.
• A Bachelor's Degree in Physical Therapy or a related field is essential.
