Key Responsibilities\n

• Collaborate with a multidisciplinary team to assess patients’ readiness for discharge, ensuring a comprehensive understanding of each individual’s discharge planning needs.

• Conduct post-discharge follow-ups to track patient progress and adjust care plans as necessary.

• Facilitate one-on-one therapy sessions, group treatments, and family psychoeducational workshops, utilizing evidence-based practices tailored to diverse client backgrounds.

• Develop personalized treatment plans and participate in ongoing treatment planning meetings to ensure the highest quality of care.

• Continuously assess the safety and wellbeing of patients, addressing any immediate safety concerns and documenting observations accurately.

• Identify potential risk factors and implement targeted interventions for patients requiring special precautions, such as those at risk for suicide or self-harm.

• Maintain thorough and timely documentation of assessments, treatment sessions, and patient progress in compliance with legal and ethical standards.
