Work Environment

This position is onsite in our outpatient office setting, which provides a comfortable, well-equipped environment for both patients and staff. Key Responsibilities

• Conduct comprehensive evaluations and assessments to determine patients' physical conditions and limitations.
• Design and implement individualized treatment plans aimed at promoting recovery, improving mobility, and enhancing overall physical function.
• Administer therapeutic exercises, utilizing various modalities and manual therapy techniques to address pain and physical impairments.
• Maintain detailed documentation of patients' progress, making modifications to treatment plans as needed to ensure optimal outcomes.
• Engage in collaborative discussions with interdisciplinary teams to facilitate comprehensive patient care and heightened outcomes.
• Educate patients and their families on home exercise programs, safety strategies, and lifestyle modifications necessary for continued rehabilitation success.

Patient Population / Client Focus

As a Physical Therapist in our facility, you will work primarily with diverse adult populations, including elderly residents with chronic conditions, post-surgical patients, and those recovering from strokes or other debilitating injuries. Your expertise and compassionate care will play a pivotal role in helping these individuals regain independence and improve their quality of life.

Qualifications

• Education: Successful completion of a Doctor of Physical Therapy (DPT) program or a Bachelor's/Master's degree in Physical Therapy from an accredited institution.
• Licensure: Must possess an active and unrestricted Physical Therapy license to practice in the state of New York.
