Key Responsibilities\n

As a Licensed Clinical Social Worker (LCSW) at Caring Connections, your key responsibilities will include:

• Conducting age and culturally sensitive biopsychosocial assessments to evaluate mental health, substance abuse, and medical needs within the hospice setting.

• Providing behavioral health services to individuals, couples, and families, including intake assessments, counseling sessions, diagnostic evaluations, treatment planning, and crisis intervention. Your compassionate approach will be essential in guiding families through difficult times.

• Documenting patient encounters, mental health evaluations, progress notes, and suicide risk assessments in accordance with organizational protocols and electronic health record systems.
\n
• Collaborating with healthcare providers to escalate patient needs and ensure comprehensive and continuous care.

• Providing individuals and families with referrals to local resources and community agencies, facilitating access to support services and medications, including substance abuse treatment programs as needed.

• Participating in quality improvement initiatives to enhance clinical processes and data collection efforts, ensuring the highest standard of care for our patients.

Patient Population / Client Focus\n

This position focuses on serving patients in a hospice environment, where care is provided to individuals with terminal illnesses and their families. Qualifications\n

To succeed in this role, candidates must meet the following qualifications:

• Possession of a current and valid Licensed Clinical Social Worker (LCSW) license in the state of California.

• A Master’s degree in Social Work from an accredited program.

Compensation and Benefits\n
