Work Environment\n

This position is remote, primarily conducted in a school facility setting, which provides a unique opportunity to collaborate with educational staff and mental health professionals. You will work closely with other healthcare providers to assess seniors' needs and implement care plans. Standard working hours are Monday through Friday, from 8:00 AM to 4:30 PM, with flexibility required for evening or weekend hours based on client needs. Your availability for on-call situations is also crucial, ensuring that we can address urgent client concerns promptly.

Key Responsibilities\n

• Perform thorough assessments of individual seniors’ psychosocial needs, evaluating emotional and social factors.

• Work closely with clients and their families to create personalized care plans that align with their goals and preferences.

• Provide counseling and emotional support during life transitions, helping clients to manage challenges such as health changes or relocation.

• Advocate for clients by facilitating their access to community resources and ensuring they receive the appropriate support services.

• Collaborate with an interdisciplinary team—including healthcare providers, caregivers, and educators— to deliver comprehensive and coordinated care.

• Conduct regular client visits to monitor physical and emotional well-being, assess living conditions, and identify any risks.

• Assist in communication among family members regarding care strategies and facilitate conflict resolution as necessary.

• Educate clients and families on available support groups, community programs, and resources that promote social engagement.

• Maintain detailed records of client interactions, assessments, and care plans, ensuring documentation is accurate and up-to-date.

Qualifications\n

Compensation and Benefits\n
