Key Responsibilities\n

• Engage with clients through virtual consultations, providing tailored counseling services to individuals and families coping with grief, loss, and various psychosocial challenges.

• Conduct thorough psychosocial assessments and develop individualized care plans that align with each client’s unique needs, goals, and treatment preferences.

• Provide case management services, which include connecting clients with community resources, support groups, and other services that enhance their overall well-being.

• Collaborate closely with the practitioner team to identify and de-escalate any risks, ensuring the safety and well-being of clients, particularly those exhibiting signs of distress or crisis.

• Lead the escalation process for emergency services and mandatory reporting according to established protocols, ensuring compliance with legal and ethical standards.

• Continually assess safeguarding risks and communicate these to clinical leads while actively contributing to mitigation strategies.

• Implement and uphold safeguarding policies within daily service delivery and proactively address any challenges to their implementation.

• Collect and analyze feedback on client outcomes, utilizing insights to drive continuous improvement in service delivery and make necessary recommendations for organizational changes.
