Work Environment\n

The position is set in a remote work environment, offering flexibility and comfort while fostering collaboration with other healthcare professionals. You will be part of a climate-controlled office setting in a hospice care facility, which encourages daily communication and teamwork among departments. You’ll have the resources necessary to conduct individual and family therapy sessions effectively, maintaining a focus on patient progress and comfort.

Key Responsibilities\n

• Conduct thorough psychosocial assessments and evaluations to understand patients' mental health challenges, including diagnostics using established criteria such as the DSM-5.

• Develop and implement individualized treatment plans in collaboration with patients and their families, ensuring cultural competence and adherence to patient needs and goals.

• Utilize evidence-based practices to facilitate ongoing patient care, ensuring the effectiveness of interventions by monitoring progress and making necessary adjustments.

• Maintain accurate and timely documentation of treatment records, adhering to clinical protocols and regulatory requirements.
\n
• Provide education and support to patients, linking them with necessary resources or referral options to enhance their treatment experience and outcomes.

• Work in a collaborative team environment, encouraging feedback and professional growth opportunities through peer reviews and clinical dialogues.

• Mentor and guide interns and volunteers, fostering a learning environment that encourages skill development and professional growth.

Patient Population / Client Focus\n

Your primary focus will be on individuals and families affected by cancer. These patients often face a myriad of social, emotional, and psychological difficulties. As a compassionate social worker, you will play a pivotal role in empowering patients by providing them with the tools and strategies they need to cope with diagnosis, treatment, and its aftermath.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current and valid State License as a Licensed Clinical Social Worker (LCSW).

• Proven experience in mental health care, preferably within a similar outpatient or hospice care setting.

Preferred Experience\n

• Experience working with cancer patients or in hospice care is highly desirable.

• Familiarity with telehealth practices and virtual care methodologies is a plus.

Compensation and Benefits\n

Stay motivated by a competitive salary range of $90,000 to $105,000 annually. In addition, our compensation package includes:
