Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing medical histories, performing assessments, and documenting your findings to develop tailored treatment plans.
• Effectively communicate evaluation results and treatment objectives to both patients and their families, involving them in goal-setting to enhance adherence and improve outcomes.
• Implement treatment protocols with a high degree of proficiency, adapting techniques based on continual assessment and evolving patient needs.
• Collaborate with interdisciplinary team members to discuss patient progress, recognizing any changes in condition that may require modifications to the care plan.
• Oversee the discharge planning process and ensure thorough follow-up care, maintaining accurate documentation throughout each patient's treatment journey to reflect their progress and needs.

Qualifications

Compensation and Benefits
