Key Responsibilities

The Physical Therapist will be responsible for:

• Directing physical therapy treatments aimed at preventing disabilities, relieving pain, and restoring optimal muscle function.
• Supervising Physical Therapy Assistants to ensure adherence to the individualized plan of care.
• Conducting thorough evaluations of clients' functional levels through various diagnostic tests and home assessments.
• Implementing prescribed physical therapy services by a physician in clients' homes and making recommendations for enhancing independence.
• Administering therapeutic exercises, traction, and massage techniques tailored to client needs.
• Assessing and adjusting prosthetic and orthotic devices while consulting with the client's physician regarding modifications.
• Participating in the multidisciplinary care team to develop, review, and continuously update client care plans.
• Documenting client progress and any changes in condition, reporting findings to the Case Manager and physicians.
• Preparing comprehensive clinical records, including evaluations, progress notes, and discharge summaries.
• Educating clients and their families about the use of therapy devices and overall home therapy programs.

Patient Population / Client Focus

This role caters primarily to a diverse patient population ranging from elderly clients requiring rehabilitation to post-surgery patients and individuals recovering from injuries. Qualifications
