Clinica Recovery LLC is actively seeking a dedicated and licensed Physical Therapist to join our team in Hackensack, New Jersey. In this full-time role, you will be pivotal in delivering personalized care to patients in the comfort of their homes. Work Environment

This role involves onsite visits to various residences within Hackensack and the surrounding areas. Working hours are Monday to Friday from 9 AM to 5 PM, providing a stable schedule that allows for work-life balance. As you travel to patients' homes, you will engage with a diverse client base, tailoring your approach to meet unique needs and circumstances.

Key Responsibilities

• Conduct comprehensive in-home assessments to evaluate each patient's physical condition, range of motion, and general functional capabilities, leading to the development of personalized care plans.
• Implement various therapeutic interventions, including but not limited to therapeutic exercises, gait training, and balance retraining, focused on improving strength and mobility.
• Educate patients and their caregivers about their treatment plans, home exercise programs, and strategies for promoting ongoing recovery and prevention of further injury.
• Collaborate closely with physicians, nurses, and other healthcare professionals to coordinate multidisciplinary care that aligns with patient goals.
• Maintain precise and timely documentation of patient progress and treatments, ensuring compliance with Medicare and agency standards.
• Travel efficiently throughout your assigned territory, managing your time effectively to see patients within scheduled appointments.

Patient Population / Client Focus

Your caseload will include a variety of patients, from those recovering from surgery to seniors needing rehabilitation due to chronic illnesses or mobility issues. Each client presents unique challenges and opportunities for you to make a meaningful impact on their recovery journey. Qualifications

• Current licensure as a Physical Therapist in New Jersey.
• Graduation from an accredited Physical Therapy program (DPT or equivalent).
• A minimum of 2 years of experience in physical therapy, specifically in a home health setting is preferred.
• Possession of a valid driver's license, reliable transportation, and an ability to travel to various patient locations.
• Strong communication, time management, and interpersonal skills, enabling effective interaction with patients, families, and the healthcare team.
• Capability to work independently while also being part of a collaborative team-oriented environment.

Preferred Experience

• Experience in home health care, particularly working with geriatric populations, is highly desirable.
• Familiarity with Medicare service standards and documentation requirements.
• Additional certifications in specialized therapy techniques (e.g., vestibular rehab, orthopedics) are beneficial.

Compensation and Benefits

This position offers a competitive salary ranging from $110,000 to $125,000 annually, complemented by mileage reimbursement for travel.
