Key Responsibilities\n

• Conduct in-depth psychosocial assessments through virtual interviews and comprehensive reviews of patients' medical histories to identify their emotional, social, and environmental challenges impacting recovery.

• Provide compassionate counseling and emotional support to patients and their families, focusing on adjustment to medical conditions and personal coping strategies during their recovery.

• Collaborate closely with our interdisciplinary rehabilitation team to develop tailored treatment plans that address both medical and psychosocial needs of each patient.

• Assist patients in navigating community resources, empowering them by providing access to support groups, vocational rehabilitation, and housing assistance, when necessary.

• Educate patients and families about available community resources and post-rehabilitation services to guarantee continuous support beyond their stay.

• Engage in discharge planning activities to facilitate seamless transitions for patients returning home or moving into alternative care settings, ensuring they have the tools needed for success.

Patient Population / Client Focus\n

As a member of our rehabilitation team, you will work with a diverse array of patients facing various challenges, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n

Compensation and Benefits\n

The salary for this role ranges from USD 75,000 to 100,000 annually, along with performance-based bonuses.
