Key Responsibilities\n

• Conduct thorough psychosocial assessments of pediatric patients and their families to identify emotional, social, and environmental strengths and weaknesses associated with their medical diagnoses and life situations.

• Utilize a variety of therapeutic interventions, including crisis intervention, as well as brief and long-term individual, group, and family therapies, tailored to the needs of the patients you serve.

• Formulate, develop, and implement comprehensive care plans in collaboration with the health care team, actively involving the patient and family members in treatment planning.
\n
• Facilitate effective discharge planning through coordination with various healthcare stakeholders, including consulting physicians and insurance providers, ensuring smooth transitions to post-hospital care.

• Provide education, counseling, and resources to help patients and families adapt to illness, fostering resilience and coping strategies.
\n
• Take part in an on-call rotation to address after-hour concerns, ensuring continuous support for patients and families.
\n
• Deliver educational presentations and workshops for staff and community partners, showcasing best practices in psychosocial oncology.

• Engage with local community resources to develop and strengthen support for cancer survivors and their families.

Patient Population / Client Focus\n

This role primarily focuses on providing emotional and psychosocial support to pediatric patients undergoing medical treatment, particularly in school settings. You will work closely with children and adolescents facing diagnoses such as cancer, as well as challenges related to chronic illnesses and their psychosocial ramifications. Your work will empower families by providing them the tools to navigate these challenges effectively.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Colorado.

Compensation and Benefits\n
