Work Environment\n

This full-time position offers the flexibility of remote work, allowing you to provide optimal support to our clients from the comfort of your home. While most interactions will take place through telehealth services, the role may occasionally require onsite visits to local seniors, ensuring you can effectively address their needs in person. Our work schedule generally runs Monday to Friday, 8:00 AM to 4:30 PM, with flexibility required for urgent client needs and occasional evening or weekend hours.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, evaluating emotional, mental, and social factors impacting their health.

• Collaborate with clients and their families to create tailored care plans that align with their specific goals and preferences.

• Provide counseling and emotional support to seniors facing transitions, navigating challenges, and adapting to changes in their lives.

• Advocate for seniors’ rights, ensuring they have access to necessary resources within the community.

• Monitor client well-being through regular check-ins, both virtually and during in-person visits, addressing any urgent safety issues or concerns.

• Facilitate communication among family members, helping to resolve conflicts related to care decisions.

• Document client assessments, care plans, and progress notes accurately using electronic health record (EHR) systems.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.
