Key Responsibilities\n

As our Senior LCSW, your primary responsibilities will include:

• Conducting comprehensive clinical assessments for patients entering hospice care, considering their psychological, emotional, and psychosocial needs.

• Developing tailored treatment plans utilizing evidence-based modalities, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), to address diverse mental health concerns.

• Documenting clinical interactions in a timely and accurate manner, ensuring compliance with HIPAA regulations and organizational policies.

• Collaborating with physicians, psychiatrists, nurses, and other caregivers to formulate a holistic approach to patient care, participating in case management meetings regularly.

• Providing educational resources and support to patients and families regarding mental health, treatment options, and available community resources, making an impact on their emotional well-being.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
