Key Responsibilities\n

• Coordinate and conduct home visits as required for comprehensive treatment plans, conducting crucial assessments and data analysis.

• Ensure the fidelity and effectiveness of program plans, implementing changes as needed to enhance patient outcomes.

• Engage in interdisciplinary team meetings to collaborate on patient care strategies, sharing insights and recommendations.

• Document monthly and quarterly progress notes, providing clear, actionable updates on each patient's journey.

• Design and facilitate training programs to ensure all staff members are equipped to implement treatment plans consistently.

• Maintain organized records, allowing for seamless information sharing and access among stakeholders.

• Expand community outreach by acting as a liaison between Invasive Care Consultants and local mental health organizations, fostering collaborative initiatives.
