Key Responsibilities\n

• Deliver high-quality psychosocial assessments and interventions to patients and families in hospice care, utilizing evidence-based practices and empowering patients to express their needs and preferences.

• Collaborate with interdisciplinary teams, including physicians, nurses, and healthcare aides, to develop and implement comprehensive care plans that address the emotional, social, and spiritual needs of patients.

• Conduct family meetings to discuss care goals, treatment options, and provide emotional support as families navigate difficult health decisions.

• Actively monitor and track patients' needs and treatment plans, ensuring timely adjustments to care responses based on changing conditions.

• Facilitate access to community resources and support services that can assist families, including bereavement counseling, financial assistance, and legal considerations.

• Provide education and training to family members on caregiving techniques, health conditions, and resources aimed at improving patients’ quality of life.

Patient Population / Client Focus\n

In this role, the primary focus will be on patients in hospice care, typically those facing terminal illnesses. Qualifications\n

• Must hold a valid LICSW license in the state of Washington.

• At least three years of prior clinical social work experience with proven expertise in end-of-life care and grief counseling is required.

• Strong understanding of federal and state regulations surrounding hospice care, including Medicare and Medicaid compliance.

Compensation and Benefits\n

Unified Care NW offers a competitive salary ranging from USD 100,000 to USD 105,000 annually, commensurate with experience. Additionally, we provide a comprehensive benefits package that includes:
