Key Responsibilities\n

• Conduct thorough assessments to evaluate the needs of clients and their families, identifying emotional, social, and environmental factors that influence their well-being.

• Design and implement individualized client treatment plans that include therapeutic interventions, goal setting, and the establishment of measurable outcomes.

• Facilitate individual and group therapy sessions to support clients in emotional and psychological healing, helping them navigate end-of-life issues and bereavement.

• Collaborate closely with interdisciplinary teams—including physicians, nurses, and other social workers—to ensure comprehensive care delivery that respects the needs and wishes of patients.

• Provide education and counseling to families about hospice services and resources, helping them understand the care process and available support systems.

• Maintain accurate and timely documentation of client interactions and treatment plans in compliance with regulatory standards and organizational protocols.

• Participate in case review meetings and contribute to quality improvement initiatives within the organization to enhance service delivery.

In this role, you will primarily work with patients in hospice care, including individuals facing terminal illnesses and their families. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• An active LICSW license in Washington State with a clear disciplinary record is mandatory.
