Key Responsibilities\n

• Conduct comprehensive assessments to determine the needs and goals of clients, formulating individualized treatment plans in collaboration with them.

• Facilitate effective communication among clients, family members, guardians, and the broader healthcare team to ensure an integrated approach to care.

• Organize and lead home visits and follow-ups to maintain continuity of care, ensuring clients have access to necessary resources and support.

• Participate actively in Interdisciplinary Team Meetings to share knowledge, insights, and updates on client progress, enhancing teamwork and collaborative efforts.

• Evaluate treatment outcomes by completing detailed monthly and quarterly progress notes, documenting client advancement, and adjusting plans as needed.

• Implement training sessions for staff on program protocols and documentation practices, ensuring that the entire team is skilled in providing quality care.

• Act as a liaison between clients and the local mental health community, advocating for services and resources that align with client needs.

Patient Population / Client Focus\n

The ideal candidate will engage with a diverse range of clients, enabling them to address a variety of mental health challenges, including anxiety, depression, family dynamics, trauma, and substance misuse. Our patient population includes both adults and youth, offering the LPC the opportunity to utilize their skills across age groups and backgrounds. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Must possess an active and unrestricted Professional Counselor (LPC) license in New Jersey.

• A minimum of two years of counseling experience in a clinical setting, demonstrating effective treatment delivery and client engagement.

• Knowledge of and specialization in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is preferred.

• Strong understanding of legal and ethical standards in mental health practice.

• Exceptional communication skills, allowing for rapport-building with a wide range of clients, families, and team members.

Preferred Experience\n

Experience working in outpatient settings and familiarity with telehealth practices are highly valued as we expand our service offerings to meet the needs of our clients efficiently. Additionally, candidates with training in trauma-focused therapy or certifications in specialized counseling techniques will be given preference, as they enhance our capacity to provide comprehensive care.
