Key Responsibilities\n

As a Licensed Clinical Social Worker, you will be responsible for:

• Conducting thorough assessments to determine clients' mental health needs and developing personalized treatment plans that support their recovery journeys.

• Documenting medical necessity and maintaining accurate records to support client entry into our programs and ensure seamless continuity of care.

• Researching, referring, and advocating for essential community resources such as healthcare services, financial assistance, and housing support to improve clients' overall well-being.

• Responding effectively to crisis situations, including child abuse and mental health emergencies, providing immediate interventions and follow-up care as needed.

• Monitoring and following up on clients' progress to assess the effectiveness of treatment plans, make adjustments as necessary, and provide ongoing support and encouragement.

• Identifying and engaging with communities in need, conducting outreach initiatives to raise awareness about available mental health services.

Qualifications\n
