Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify and address the specific mental health concerns faced by clients and their families within a hospice setting.

• Deliver high-quality individual, family, and group therapy utilizing evidence-based modalities tailored to the end-of-life care process.

• Develop and implement personalized treatment plans that reflect clients' needs, goals, and available resources, regularly reassessing effectiveness throughout the treatment process.

• Engage in collaborative practices by working alongside psychiatrists, psychologists, and other healthcare providers to ensure comprehensive care for your clients.

• Facilitate crisis intervention strategies to help clients and families navigate emotional distress and loss, ensuring safety and stability.

• Maintain accurate, up-to-date documentation in accordance with ethical and legal standards to support best practices in mental health care.

• Lead group therapy sessions, psychoeducation classes, and support groups aimed at helping clients cope with grief and loss.

Your work will directly impact a varied patient population, primarily consisting of individuals facing terminal illnesses and their families. You will address complex emotional experiences such as fear, grief, and anxiety, fostering resilience and emotional well-being among clients in their most vulnerable moments.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience in a mental health or outpatient setting, preferably with a focus on hospice or palliative care.
