Key Responsibilities\n

• Engage proactively with program participants to assess their needs and prevent crises, emphasizing outreach and support rather than reactive measures.

• Maintain detailed documentation of client interactions, including crisis alerts to coordinate seamlessly with local county crisis services.

• Facilitate transitions for clients moving from inpatient hospital care back to outpatient settings, collaborating with hospital staff to ensure a smooth discharge and continuity of care.

• Develop individualized treatment plans for each client, considering their unique goals, needs, and the resources available to them.
\n
• Regularly monitor client progress through follow-up assessments and adjust treatment strategies as necessary to promote optimal health outcomes.

• Participate in regular team meetings and professional development sessions to enhance knowledge and improve service delivery techniques.

• Conduct educational sessions for clients and community members to promote mental health awareness and skills for coping with mental health challenges.

Patient Population / Client Focus\n

Our counselors primarily work with diverse individuals facing a variety of mental health challenges, including but not limited to anxiety, depression, PTSD, and substance misuse. Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling is required.

• Current licensure as a Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW) in the state of Oregon.

• Proficient in using electronic health record (EHR) systems for documentation and client management.

• Strong clinical skills are essential, with an emphasis on building rapport and trust with clients.

• Excellent verbal and written communication skills for effective interaction with clients, families, and other professionals.

Preferred Experience\n

Compensation and Benefits\n

Mind Discovery Mental Health Services offers a competitive salary range of $100,000 to $105,000 per year, based on caseload and experience.
