Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients' mental health needs, identifying strengths and potential challenges.

• Utilize cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care techniques tailored to meet the unique needs of patients.

• Develop, implement, and refine individualized treatment plans in collaboration with clients and other treatment team members, ensuring all interventions are aligned with their goals.

• Engage in regular supervision and consultation with psychiatrists and psychologists to maintain coordinated care and adjust treatment strategies as necessary.

• Facilitate crisis interventions when needed, conducting risk assessments and safety planning for patients in acute distress.

• Maintain accurate clinical documentation in compliance with ethical, legal, and organizational standards.

• Organize and lead group therapy sessions and psychoeducational classes, fostering a sense of community among clients.

Patient Population / Client Focus\n

Your primary focus will be on supporting individuals receiving hospice care, many of whom are grappling with significant psychological distress due to terminal illness and end-of-life issues. Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Active and unrestricted state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
