Key Responsibilities\n

• Utilize your clinical expertise to conduct thorough psychosocial assessments, create independent diagnoses, and develop individualized treatment plans tailored to each patient's needs.

• Engage with clients through teletherapy or in-person visits, applying evidence-based techniques such as solution-focused therapy, behavioral activation, and motivational interviewing to enhance treatment outcomes.

• Maintain consistent communication with a multidisciplinary team, including medical providers and therapists, to ensure holistic care across all touchpoints.

• Systematically monitor patient progress and adjust treatment plans as necessary, utilizing electronic health record (EHR) systems to document care and outcomes effectively.

• Assess and support patients in managing psychotropic medications, focusing on adherence, potential side effects, and overall effectiveness, while providing necessary recommendations to prescribing physicians.

Your primary focus will be on patients receiving hospice and palliative care, often facing serious or terminal illnesses. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current, valid, and unrestricted license to independently practice as a Licensed Clinical Social Worker (LCSW) in Georgia.

• Minimum of 1 year of post-graduate clinical experience working with patients in a therapeutic capacity.
