Work Environment\n

Key Responsibilities\n

• Utilize your education and clinical experience to conduct thorough psychosocial assessments, delivering accurate independent diagnoses to patients.

• Collaborate with patients to design patient-centered treatment plans that prioritize their individual needs, preferences, and goals.

• Administer evidence-based interventions, including solution-focused therapy and motivational interviewing, to facilitate patient engagement and progress.

• Work closely with interdisciplinary teams, including psychologists, primary care providers, and other clinicians, to provide a holistic approach to patient care.

• Monitor and evaluate patients' progress through regular check-ins, whether in person or virtually, and adjust treatment plans as needed to ensure effectiveness.

• Support medication management for patients receiving psychotropic medications, managing adherence, side effects, and efficacy through collaboration with medical providers.

Patient Population / Client Focus\n

In this role, you will primarily work with patients within the hospice care setting, assisting individuals and families coping with terminal illnesses. Your approach will be centered around empathy and understanding, integrating emotional support with practical guidance to help patients and their families during one of the most challenging times of their lives. You will also serve as an advocate for their needs, ensuring they have access to the resources and support necessary throughout their journey.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• A current and valid LCSW license permitting independent practice in mental health within the state of Florida.

• A minimum of one year of experience as an independently licensed therapist, demonstrating competence in clinical decision-making and patient interaction.

Compensation and Benefits\n
