This role is fully remote, allowing LCSWs the flexibility to provide services to clients at their convenience while still maintaining a strong connection with colleagues and support systems within our Community Health Clinic. Key Responsibilities\n

• Conduct thorough assessments to evaluate the therapeutic needs of patients, employing evidence-based practices and using standardized assessments.

• Utilize the DSM (Diagnostic and Statistical Manual) criteria to diagnose clients for substance use disorders, identifying appropriate care levels according to ASAM (American Society of Addiction Medicine) guidelines.
\n
• Develop and prepare individualized treatment plans that address the specific substance abuse counseling needs of each client, adapting approaches as required to optimize effectiveness.
\n
• Create structured discharge and continuity of care plans to facilitate transition to the next appropriate level of care post-treatment.
\n
• Maintain detailed documentation of all counseling interactions, educational services, assessments, treatment plans, and follow-ups within the client's clinical record.
\n
• Manage a designated caseload effectively, conducting regular utilization reviews and ensuring that treatment and progress are tracked accurately.
\n
• Coordinate support services for both patients and their families, being sure to document this support in clinical records for transparency and continuity.
\n
• Engage with external referral sources such as legal representatives, educational institutions, and employers, ensuring a collaborative effort in client support.
\n
• Address crisis situations promptly, employing a multidisciplinary approach to develop interventions for assigned clients, maximizing their therapeutic outcomes.

Patient Population / Client Focus\n

The LCSW will primarily work with patients experiencing co-occurring mental health and substance use disorders. Qualifications\n

• A Bachelor's degree in Social Work from an accredited institution.

Preferred Experience\n

Compensation and Benefits\n
