Key Responsibilities\n

• Conduct thorough psychosocial assessments through patient interviews, clinical observations, and comprehensive review of patient medical records to identify key emotional and social determinants affecting patient recovery.

• Provide empathetic counseling and emotional support to patients and their families, addressing adjustment issues related to serious illness and end-of-life considerations.

• Collaborate with the interdisciplinary rehabilitation team to formulate personalized treatment plans tailored to the unique psychosocial needs of each patient.

• Facilitate access to community resources such as support groups, vocational rehabilitation services, and financial assistance programs to support patients and families during this critical time.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

Compensation and Benefits\n

We offer a competitive annual salary ranging from $70,000 to $100,000, with additional performance-based bonuses.
