Key Responsibilities\n

• Conduct individual counseling and therapy sessions aimed at improving clients’ mental health and coping strategies, utilizing evidence-based treatments relevant to hospice care.

• Carry out comprehensive psychosocial assessments to inform treatment planning, ensuring that each client receives tailored interventions that align with their unique emotional and psychological needs.

• Develop and implement detailed treatment plans, maintaining thorough and accurate records, including clinical and progress notes, in compliance with department standards.

• Collaborate closely with a multidisciplinary team—including physicians, nurses, and chaplains—to design and adjust treatment plans, including effective discharge strategies.

• Provide crisis intervention and support, addressing emotional and behavioral issues that arise during the sensitive end-of-life process.

• Facilitate communication and coordination with clients’ families and support systems regarding treatment options and discharge plans, ensuring all parties are informed and supported.

Patient Population / Client Focus\n

The clients served by our hospice center primarily include individuals facing terminal illnesses and their families, requiring specialized emotional support to navigate the challenges of end-of-life care. Your expertise as an LCSW will be invaluable in providing grief counseling, coping strategies, and emotional stabilization for clients experiencing a myriad of feelings including fear, anxiety, and sadness.

Qualifications\n

• A Master’s degree in Social Work or Counseling from an accredited institution.

• Valid and active state license as a Licensed Clinical Social Worker (LCSW).

• At least 3 years of experience in an outpatient mental health setting, particularly focusing on hospices or palliative care environments.

• Proven ability to work independently, demonstrating self-direction and exceptional organizational skills.

• Proficiency in Microsoft Office Suite and electronic health record systems.

Compensation and Benefits\n
