Key Responsibilities\n

• Serve as a vital advocate for clients, assisting them in navigating community resources, social services, and support systems that enhance their quality of life.

• Maintain and organize accurate client records, progress notes, and treatment plans, ensuring adherence to legal and ethical standards.

Patient Population / Client Focus\n

You will primarily work with adults facing complex and often challenging circumstances often related to end-of-life issues, coping with chronic illness, or navigating grief and loss. Your role will be crucial in providing empathetic support to clients and their families, helping them maintain dignity and peace during their time at the Hospice Center.

Qualifications\n

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Tennessee.

• A Master’s degree in Social Work (MSW) or related discipline is required.

• Minimum of four years of relevant experience as a social worker, preferably in an outpatient or hospice setting.

Compensation and Benefits\n
