Key Responsibilities\n

As a Licensed Professional Clinical Counselor, you will:

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, actively contributing to an integrated care team.

• Conduct comprehensive patient intakes that include diagnostic evaluations and functional assessments, providing recommendations that guide treatment plans.

• Deliver brief but focused interventions tailored to patients in need of immediate mental health services.

• Evaluate patient cases to determine the appropriateness of continued care in primary settings versus referrals to specialized mental health facilities.

• Implement follow-up procedures to monitor patient progress and adjust treatment approaches as necessary.

• Formulate and develop relapse prevention strategies and treatment plans to help patients achieve stable functioning.

• Detect at-risk patients and collaborate on plans aimed at preventing deterioration of their mental health status.

• Coordinate with relevant service providers, ensuring seamless delivery of healthcare services tailored to the behavioral health needs of patients.

• Document patient progress accurately in treatment records while ensuring confidentiality and compliance with ethical standards.

• Provide patients with the education and self-management strategies essential for active participation in their recovery journey.

• Engage in team discussions, advising on behavioral health services and contributing to program evaluation and outcomes.

• Facilitate compliance with prescribed medical treatments from primary care providers, including managing any medication-related side effects.

Qualifications\n

To excel in this role, candidates should possess:

Preferred Experience\n
