Position Overview

Kinetic Revive Physical Therapy is actively seeking a dedicated and experienced Physical Therapist (PT) to join our skilled nursing facility team located in Portsmouth, Virginia. This full-time position offers the opportunity to positively impact the lives of residents by providing top-tier therapy services aimed at enhancing mobility, alleviating pain, and promoting overall wellness. The successful candidate will work from Monday to Friday, from 9 AM to 5 PM, ensuring all residents receive optimal therapy interventions throughout the week.

Work Environment

The Physical Therapist will work onsite within our state-of-the-art rehabilitation center in Portsmouth. Key Responsibilities

• Conduct thorough assessments and evaluations of residents' physical abilities, utilizing a variety of assessment tools and techniques to determine individual therapy goals.
• Create and implement individualized treatment plans that are evidence-based and focused on restoring mobility, enhancing functional independence, and alleviating pain.
• Provide hands-on therapy, deploying therapeutic exercises, balance training, and mobility interventions tailored to the specific needs of each resident.
• Educate patients, families, and caregivers about techniques for safe mobility and therapeutic exercises that can be practiced at home.
• Work collaboratively with a multidisciplinary team, including nurses and other allied health professionals, to ensure comprehensive and cohesive patient care.
• Maintain detailed and accurate documentation of treatment plans, progress notes, and outcomes, adhering to all SNF regulations and standards.

Patient Population / Client Focus

This role primarily serves geriatric patients who may be facing complex health conditions requiring rehabilitative therapy. Our residents often include individuals recovering from surgeries, strokes, orthopedic injuries, and other ailments that hinder mobility. As a Physical Therapist, your expertise will be crucial in helping these residents regain their independence and improve their quality of life, making a profound impact on their rehabilitation journeys.

Qualifications

• A Doctorate or Master's degree in Physical Therapy from an accredited institution is required.
• Current and valid Virginia Physical Therapist license in good standing.
• Strong communication skills, with an emphasis on interpersonal relations and patient-centered care.
• The ability to thrive in a fast-paced environment and work effectively within a multidisciplinary healthcare team.

Preferred Experience

• At least 2 years of experience in skilled nursing, geriatric care, or long-term rehabilitation settings is preferred.
• Experience with rehabilitation processes involving post-operative care and chronic illness management is a plus.
• Familiarity with documentation requirements and regulatory standards for SNFs.

Compensation and Benefits
