Key Responsibilities\n

• Maintain open lines of communication with supervisors, healthcare staff, family members, and relevant stakeholders to provide updates on client progress and adapt treatment strategies accordingly.

• Develop and share a well-planned schedule for client sessions, virtual home visits, and necessary follow-ups to monitor treatment effectiveness consistently.

• Facilitate in-home assessments through video calls and virtual check-ins, ensuring a thorough understanding of each client's living situation as part of their treatment strategy.

• Monitor the implementation of individualized treatment plans, ensuring adherence to best practices and organizational protocols.

• Actively participate in interdisciplinary team meetings to discuss client progress, share insights, and collaborate with fellow professionals on complex cases.

• Document clinical notes and progress reports on a monthly and quarterly basis, maintaining comprehensive records that support ongoing treatment and compliance.

• Organize training sessions for your peers to enhance team competence in implementing therapeutic techniques and accurate documentation.

• Conduct routine follow-ups to ensure continuity of care, which may include virtual home visits and additional observational assessments.

Qualifications\n
