Key Responsibilities

• Treatment Planning: Collaborate with patients and their families to discuss evaluation findings. Set achievable goals and adapt treatment protocols based on individual progress and the evolving healthcare landscape.
• Interdisciplinary Collaboration: Work closely with other healthcare professionals to provide coordinated care. Recognize any changes in a patient's condition and promptly revise their plan of care as needed.
• Documentation: Maintain precise and timely records of patient encounters, including initial evaluations, daily notes, interim progress updates, and discharge summaries that reflect each patient's journey through care.
• Patient Discharge: Participate in discharge planning to ensure patients have a clear understanding of their follow-up care and ongoing rehabilitation needs.

Qualifications
