Key Responsibilities\n

• Conduct comprehensive patient evaluations to identify psychosocial needs and develop individualized care plans tailored to each patient's specific situation.

• Maintain detailed and compliant documentation, ensuring that all patient interactions and care interventions meet regulatory standards and organizational policies.

• Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate holistic care, ensuring that physical, emotional, and spiritual needs of the patient are met.

• Educate patients and families on available resources and coping strategies, equipping them with tools to navigate the complexities of chronic illness and end-of-life planning.

Patient Population / Client Focus\n

This role primarily serves patients in hospice care who are facing terminal illnesses. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required, along with a current LCSW license to practice in California.

Compensation and Benefits\n
