Key Responsibilities\n

• Conduct thorough psychosocial assessments of clients and their families at various stages of engagement, using effective social work theories and interventions to identify emotional and social needs.

• Provide both individual and group clinical supervision for Master’s level social workers and student interns, guiding their professional development and ensuring high standards of care.

• Prepare and maintain comprehensive electronic medical records according to departmental guidelines, delivering regular written and oral reports to ensure continuity of care.

• Collaborate with interdisciplinary healthcare teams to develop and implement personalized psychosocial treatment plans, coordinating referrals to various specialty and social services as needed.

• Assist clients and families in understanding and coping with diagnosis-related issues by providing psychoeducation and addressing barriers to care.

• Manage a diverse caseload independently, utilizing effective counseling techniques to address varying clinical acuity and needs.

• Engage in community outreach and case finding initiatives to identify clients with potential psychosocial needs, actively participating in team conferences and interdisciplinary rounds.

• Deliver supportive counseling, grief counseling, crisis intervention, and therapeutic services, empowering clients to cope with challenges associated with illness, medical transitions, and changes in care levels.

• Contribute to quality assurance initiatives and participate in the development of policies and procedures related to social work services.

• Provide mental health diagnoses and facilitate comprehensive treatment plans, ensuring each client receives the appropriate support tailored to their specific circumstances.

Qualifications\n

• A Master’s degree in a behavioral science field or a related area from an accredited institution.
