Key Responsibilities\n

• Conduct comprehensive assessments of clients to identify their unique mental health needs and develop personalized treatment plans.

• Facilitate effective communication among supervisors, direct care staff, family members, and guardians regarding client progress and treatment plans.

• Provide a tentative schedule for appointments and follow-up visits, ensuring consistent monitoring of each client's progress.
\n
• Coordinate home visits (when necessary) to conduct in-home assessments, chart reviews, and interviews, delivering tailored support based on findings.
\n
• Ensure the effective implementation of treatment plans, regularly reviewing and adjusting the plans as client needs evolve.
\n
• Actively participate in Interdisciplinary Team Meetings, contributing insights into client care and treatment strategies as required.
\n
• Document client progress through detailed monthly and quarterly notes, maintaining accurate and accessible client records.
\n
• Organize training for staff members, ensuring they are well-prepared in implementing treatment plans and utilizing documentation protocols.
\n
• Maintain professionalism by acting as a liaison between the public and the mental health community, promoting understanding and engagement in mental health initiatives.

• Assist in developing training materials and processes to continuously improve programs tailored to individual client needs.

Qualifications\n

• A Master’s or Doctorate degree in Counseling, Psychology, or a related field.

• Must possess an active and unrestricted Professional Counselor (LPC) license in New Jersey.
