Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, considering emotional, mental, and social factors that influence their overall health.

• Collaborate with clients and their families to develop tailored care plans that address individual goals and preferences.

• Provide counseling and emotional support to seniors and their families, assisting them through life transitions, such as terminal illness and end-of-life decisions.

• Advocate for the rights of clients, ensuring they have access to vital community resources, healthcare services, and support groups.

• Work in partnership with nurses, caregivers, and other healthcare professionals to coordinate and enhance comprehensive care delivery.

• Regularly monitor clients' well-being through virtual consultations and in-home visits, offering guidance on safety, living conditions, and emotional support.

• Facilitate family communication, helping to resolve conflicts and foster collaboration in client care.

• Educate clients and their families on available community resources aimed at enhancing social engagement and quality of life.

• Maintain meticulous documentation of client interactions, care plans, progress, and any pertinent updates, ensuring compliance with regulatory standards.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n

We offer a competitive salary range of USD 100,000 – 120,000 annually, commensurate with experience.
