Work Environment\n

As a member of our outpatient mental health team, you will work in a supportive and collaborative environment designed to promote both the professionals' and patients' well-being. While the majority of your work can be performed remotely, you will have access to our office facilities in Rockford for occasional in-person meetings and training sessions. Key Responsibilities\n

• Conduct comprehensive assessments to understand the unique needs and challenges faced by each patient, tailoring interventions accordingly.

• Regularly communicate with supervisors, collaborating team members, family members, and other stakeholders to discuss patient progress and create an effective support network.

• Develop and maintain a schedule for appointments, home visits, and progress tracking to ensure continuity of care and accountability.

• Coordinate home visits and engage in in-home observations, chart reviews, and data analysis to meet program requirements.
\n
• Implement individualized program plans, ensuring they are executed with fidelity to maximize therapeutic outcomes.

• Actively participate in interdisciplinary team meetings, contributing insights and strategies that enhance patient care.
\n
• Compile and complete monthly and quarterly progress notes to document patient advancements and program effectiveness.
\n
• Organize training sessions for staff, fostering competence in program implementation and documentation practices.
\n
• Provide follow-up support through interviews, home visits, and observations to address ongoing patient needs.

• Maintain organized records that keep all relevant information readily available for patient care and assessments.

• Serve as a liaison within the mental health community to facilitate the formation of collaborative relationships focused on individual development goals.

• Assist in creating and refining training materials that aim to enhance program effectiveness for individuals.

Qualifications\n
