Key Responsibilities\n

• Conduct thorough and ongoing patient assessments to tailor mental health strategies that meet individual needs.

• Maintain open communication channels with supervisors, co-workers, families, and other relevant parties to discuss client progress and treatment plans.

• Schedule and manage appointments, home visits, and clinical observations, providing supervisors with a clear outline of ongoing activities.
\n
• Coordinate and perform home visits, including chart reviews and staff interviews, to provide comprehensive care and program evaluations.
\n
• Ensure all treatment plans are implemented effectively and adjusted as necessary, drawing on data insights and patient feedback.
\n
• Take an active role in Interdisciplinary Team Meetings, sharing insights and collaborating on patient care strategies.
\n
• Document monthly and quarterly progress notes to accurately track and report patient progress against established goals.
\n
• Organize and lead training sessions to certify staff members' competencies in implementing program plans and documentation procedures.
\n
• Conduct follow-up interviews and observations to maintain ongoing support for clients' evolving needs.
\n
• Serve as a liaison between the community and mental health services, fostering relationships and ensuring continued advocacy for client goals.

• Assist in developing training materials and procedures aimed at improving program effectiveness for clients.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.

• Proficiency in EMR software and telehealth-related technologies for efficient client management.

• Demonstrated ability to work collaboratively within a healthcare team.
