Key Responsibilities\n

• Conduct initial patient intakes focusing on comprehensive diagnostic and functional evaluations, ensuring that treatment recommendations align with each patient's unique needs.

• Collaborate with primary care providers to assess and manage mental health and psychosocial issues, incorporating brief interventions tailored to patients in need.

• Develop and implement relapse prevention plans when indicated, providing ongoing support to help patients maintain stability in their mental health.

• Monitor patient progress continuously, coordinating care with other treatment providers, while adhering to established Agency and Federally Qualified Health Center (FQHC) guidelines.

• Document clinical interactions thoroughly, maintaining accurate and up-to-date patient records in compliance with professional standards.

• Provide consultation and education to clinic management and interdisciplinary team members regarding behavioral health strategies and service outcomes.

• Advise on referrals for patients requiring specialty mental health services, ensuring they receive the most appropriate level of care.

Qualifications\n

Compensation and Benefits\n

The position offers a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience. Additionally, a SIGN-ON BONUS is available to qualified candidates.
