Key Responsibilities

• Conduct thorough Physical Therapy evaluations for pediatric and adult patients to assess their functional capabilities and rehabilitation needs.
• Develop and implement individualized treatment plans that align with each patient's personal goals, medical conditions, and treatment progress.
• Regularly assess clinical progress through observational and measurable outcomes to ensure the effectiveness of treatments, making necessary adjustments based on patient responses.
• Maintain accurate and timely documentation of patient interactions, progress, and treatment plans using our electronic record system, adhering to regulatory standards and company policies.
• Educate patients and their families regarding the recovery process to ensure understanding and address any concerns about expected challenges.
• Achieve and maintain minimum patient utilization and quality metrics as established by management.
• Work collaboratively with other health care disciplines to streamline the care process for patients receiving multiple services.
• Actively promote a positive workplace culture by exemplifying Heal Motion Care's core values and serving as a role model for teamwork and community engagement.

Patient Population / Client Focus

As a Physical Therapist at Heal Motion Care, you will have the opportunity to work with a diverse patient population, including children with developmental challenges, adults recovering from surgery, and seniors dealing with chronic pain or mobility issues. You will be instrumental in helping these individuals navigate their challenges through tailored therapeutic approaches designed to optimize recovery and enhance their daily lives.

Qualifications

• A Doctor of Physical Therapy (DPT) degree from an accredited institution is required.
• Possession of current, unrestricted license to practice Physical Therapy in California.
• Flexibility to conduct evaluations and treatments in-person as well as through telehealth platforms, when clinically appropriate.
• A genuine passion for working with diverse patients, embracing differences in age, race, and cultural backgrounds.
• A demonstrated ability to adapt quickly within a dynamic environment, showing resilience during fluctuating patient loads.
• Proven team-oriented collaboration skills with a focus on delivering exemplary customer service.

Preferred Experience

• Experience working in various rehabilitation settings, such as outpatient clinics, inpatient facilities, or home health.
• Familiarity with electronic health record systems and documentation practices.
• Background in pediatric therapy or neurological rehabilitation is a plus.

Compensation and Benefits
