Key Responsibilities\n

• Perform comprehensive psychosocial assessments by conducting interviews and observations, as well as reviewing relevant medical records to better understand each patient’s emotional, social, and environmental factors that could influence their rehabilitation.

• Deliver counseling and emotional support to patients and families, focusing on adjustment issues related to medical conditions, grief, and coping mechanisms.
\n
• Collaborate closely with an interdisciplinary rehabilitation team, participating in the formulation and adjustment of individualized treatment plans that consider psychosocial factors affecting recovery.

• Facilitate access to community resources for patients, providing knowledge about support groups, vocational rehabilitation, housing assistance, and post-rehabilitation services.

• Engage in discharge planning processes to ensure smooth transitions for patients returning home or moving to other care settings, including follow-up care recommendations.

Patient Population / Client Focus\n

In your role as an LCSW, you will primarily serve a varied patient population requiring psychological and emotional support following significant life changes due to illness or injury. Your expertise will be vital in helping patients adapt to new realities, cope with potential grief, and improve their overall well-being, promoting successful reintegration into their communities.

Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW) .
